The following information will be helpful if you will be seeing us for your first visit, or want to become familiar with our office policies and procedures.
If you are a new patient to our practice and have yet to have your appointment, please click on New Patient Package; download, print and complete the forms and bring them with you to your first appointment.
Please come to your appointment at least 30 minutes before your scheduled time. If you need to cancel an appointment, please call at least 24 hours before your scheduled appointment. You may be charged $25 for a missed appointment if you do not notify us 24 hours prior to your scheduled appointment time. Help us serve you better by keeping your regularly scheduled appointment, if possible.
If you are over 15 minutes late for your appointment, you may be asked to reschedule your appointment.
For your first office visit with us, plan to spend at least one hour with us. This will allow your physician to get a complete medical history from you and perform a physical examination. Diagnostic testing may also be performed at this time, or possibly at a later date. Appropriate follow-up care will be discussed and planned at your initial visit.
Be sure to bring the following with you:
- Your current medical insurance cards
- All current medications in their bottles or an up-to-date medication list, including any over-the-counter medications you are taking
- Medical records from your referring physician pertaining to your current condition.
- Completed New Patient Package forms
- Insurance co-payment (insured patients) or deposit (uninsured patients) must be paid at the time of service.
We participate in most insurance plans, including Medicare. If you are not insured by a plan we do business with, payment in full is required at each visit. If you are insured by a plan we do business with, but don’t have an up-to-date insurance card, payment in full for each visit may be required until coverage is verified. Knowing your insurance benefits is your responsibility.

- Lab Results
Please allow a minimum of three (3) working days for the results of your lab tests to come back to us from the lab. If you have a scheduled appointment, your results will be discussed with you at the time of your visit. If you do not have a scheduled appointment, your bal results will be reviewed by our clinical staff and a letter will be mailed to you.
- Call Backs
Our nurses make their call backs in the order in which they are received. Please allow them enough time for them to call you back. Emergencies will be handled immediately.
- Emergencies
If you think you are experiencing a potentially life threatening situation, CALL 911. For urgent heart problems during our office hours, call our Reno office (775) 688-8000 and one of our nurses will assist you, or CALL 911. After hours, CALL 911.
* Please notify emergency room personnel that you are a SNCA patient, and have them notify the SNCA physician on call.
